The primary responsibility of the Home Care Coordinator/Community Health Nurse Per Visit is to provide and coordinate nursing services rendered to the patients of the Certified Home Health Care Program (CHHA). The Director of Clinical Services provides clinical supervision of the nursing staff.

Years/Type of ExperienceRequired:	Must hold a current New York State Registered Nurse (RN) license. Bachelors degree in Nursing or health or human services field and or Associate Degree from an approved program. Minimum of two years supervisory experience in a home health agency licensed in the State of New York or two years of medical/surgical experience required. Must possess a current NYS RN license. Must possess ability to make own transportation arrangement to referral sources either through public or private means. Must be bilingual in Cantonese or Mandarin
